![]() While CPAP in cardiogenic pulmonary oedema has been shown to reduce intubation rate, a randomized trial in acute hypoxemic respiratory failure, showed no effect of CPAP in reducing intubation rate and mortality, despite improved oxygenation. Although some studies suggest a role for non-invasive ventilation (NIV) in mild ARDS, including a recent meta-analysis, invasive mechanical ventilation remains the standard of care, especially for severe cases. In terms of clinical management, initial recommendations suggested early intubation and ARDS-type ventilator settings. The classical clinical features of ARDS after intubation such as low pulmonary compliance are not found in all patients. Disorders of the pulmonary circulation (thrombosis, endothelial injury) and organizing pneumonia may also be present.
